After Massive Weight Loss: Patients’ Expectations of Body Contouring Surgery

Kitzinger HB et al. – Surplus skin resulting from gastric bypass surgery is a common issue that causes functional and aesthetic impairments in patients. Consequently, this increases the desire for body contouring surgery with high expectations for the aesthetic outcome as well as improved life satisfaction.

Methods

  • A questionnaire addressing information on the satisfaction of body image, quality of life, and expectation of body contouring surgery following massive weight loss was mailed to 425 patients who had undergone gastric bypass surgery between 2003 and 2009.
  • Of these 425 individuals, 252 (59%) patients completed the survey.

Results

  • Ninety percent of women and 88% of men surveyed rated their appearance following massive weight loss as satisfactory, good, or very good.
  • However, 96% of all patients developed surplus skin, which caused intertriginous dermatitis and itching.
  • In addition, patients reported problems with physical activity (playing sports) and finding clothing that fit appropriately.
  • Moreover, 75% of female and 68% of male patients reported desiring body contouring surgery.
  • The most important expectation of body contouring surgery was improved appearance, followed by improved self–confidence and quality of life.

Note: I have had multiple major weight loss patients who have had tremendous improvements after face lift, neck lift, cosmetic body lift, abdominoplasty, breast lift and breast augmentation.

Minimally Invasive Otoplasty: Technical Details and Long-Term Results A. de la Fuente and G. Sordo

Aesthetic correction of the ears requires thorough diagnosis and subsequent treatment of the affected areas. The authors’ technique addresses every deformity that might need correction through separate incisions, minimal scars, and conservative dissection without the use of permanent sutures. Correction of the eventual discrepancy between cartilage height and the skin envelope is particularly observed and addressed by means of scaphal reduction. The technical details, long-term results, and complications are evaluated in this article.

Methods: A retrospective review of 100 otoplasties performed between 2005 and 2009 by a single surgeon was conducted. Charts were reviewed and data were collected on the type of deformity corrected, the complications, and the outcome.

Results : Minimally invasive otoplasty was performed for 100 patients, primarily women (n = 71), with an average age of 23.2 years. In 96 cases, bilateral correction was performed using a combination of concho-mastoid sutures (81%), scapha reduction (76%), antihelical folding (70%), conchal reduction (53%), or earlobe correction (19%). Three patients experienced partial recurrence and needed further correction. Partial upper pole necrosis developed in one patient after undue tension was applied to his dressing, and further correction was needed. Three patients experienced mild partial skin discoloration and epidermolysis that lasted no more than 10 days. Most of the patients were very satisfied (84%) or satisfied (15%) with the procedure after an average follow-up period of 12.4 months.

Conclusions : Restoration of facial harmony through aesthetic correction of the ears is a highly satisfying procedure for both patients and surgeons. Since 1988, the author’s unit has pursued and improved a technique that could provide shorter recovery times, minimal morbidity, and predictable results for a population consisting primarily of adults. This minimally invasive technique can achieve these goals with excellent long-term aesthetic outcomes.

Sensation Following Immediate Breast Reconstruction with Implants Jakob Lagergren MD, PhD1,2,Marie Wickman MD, PhD1,2, Per Hansson MD, PhD, DDS3,4 12 NOV 2010

Abstract: Sensation is a neglected aspect of the outcome of breast reconstructions with implants. The aim of this prospective study was to evaluate the cutaneous somatosensory status in breasts following mastectomy and immediate reconstruction with permanent adjustable prostheses and to analyze the patients’ subjective experience of the sensation. Twenty-four consecutive patients diagnosed with invasive or in situ breast carcinoma were examined preoperatively and 2 years after mastectomy and reconstruction, for assessment of perception thresholds for touch, cold, warmth, and heat pain above and below the areola. Von Frey filaments and a Peltier element-based thermode were used. The patients completed a questionnaire concerning their experienced sensation in the reconstructed breast. Using quantitative somatosensory testing, the sensation to all the examined modalities was significantly impaired compared to preoperatively. Most affected was the area above the areola. Patients given postoperative radiotherapy (n = 9) did not differ from those without radiotherapy (n = 15) regarding any of the modalities. All patients reported reduced sensation in the reconstructed breast compared to that preoperatively. Twenty-three patients stated that the reconstructed breast felt different from the other breast; nevertheless 16 reported that the reconstructed breast felt like a real breast. The study revealed sensation impairment following mastectomy and immediate reconstruction with implants. Patients should be informed about this effect preoperatively to allow adequate expectations regarding the sensation outcome. However, two-thirds of the study patients considered that the reconstructed breast felt like a real breast, which must be one of the main purposes of a breast reconstruction. Abstract

Background Few techniques for the correction of buttock ptosis have been described in the literature.

Objective A technique for treating buttock ptosis, as well as long or asymmetrical buttocks, by making an incision in the gluteal crease is described.

Methods With the patient standing, the redundant skin was delineated and marked by pushing the skin towards the ischial tuberosity with two fingers, and tucking in the redundant skin, creating a new crease. The hanging skin, previously marked, was de-epithelialized. A longitudinal cut was made through the dermis and subcutaneous fatty layer until the ischial tuberosity was exposed, to which the edges of the dermal flaps were anchored.

Results This technique was performed in 39 patients. The results were satisfactory, generally producing good quality scars and long-term lifting of the buttocks. The few complications were limited to immediate scarring problems; 3 cases required scar touch-up.

Conclusions The dermo-tuberal anchorage technique for treatment of buttock ptosis is simple to perform and produces good long-term results. It can be used in conjunction with lipoplasty, lipografting, and/or gluteal augmentation.

FDA approves Botox as treatment for chronic migraines.

On the front page of its Business Day section, the New York Times (10/15, B1, Singer) reported, "The Food and Drug Administration on Friday approved Botox, the anti-wrinkle shot from Allergan, as a treatment to prevent chronic migraines, a little more than a month after the company agreed to pay $600 million to settle allegations that it had illegally marketed the drug for unapproved uses like headaches for years." Botox "had worldwide sales last year of about $1.3 billion, divided equally between medical and cosmetic uses." But Allergan said "sales of Botox for chronic migraine and other medical uses would soon eclipse sales of the drug as a wrinkle smoother.
        The Los Angeles Times (10/15, Healy) "Booster Shots" blog reported, "The FDA's decision expands the potential market for Botox, which burst upon the American cosmetic scene in the late 1980s, to 12% of the US population -- the proportion of Americans thought to suffer from the throbbing, pulsating pain of migraine headaches." The approval allows Allergan "to advertise to consumers and promote to doctors the use of Botox for chronic migraine." The FDA "underscored Friday that Botox does not appear to be useful in treating or preventing less frequent migraines, or headaches that are not caused by the activation of nerve fibers within the brain's blood vessels--the definition of migraine headache."
        The AP (10/15) reported, "For the new use, doctors are directed to inject patients in the neck or head every 12 weeks to dull future headaches." The FDA "approved the new use based on two company studies of more than 1,300 patients who received either a Botox injection or a dummy injection." Those "who received Botox reported slightly fewer 'headache days' than patients given the sham treatment. In the more significant of the two studies, patients on Botox reported about two fewer headache days than patients who didn't receive the drug."
        Bloomberg News (10/15, Peterson) reported, "On Oct. 5 the company settled a 2 1/2-year investigation of its Botox marketing that analysts said held up FDA approval for the migraine treatment." Allergan "pleaded guilty and was ordered by a judge to pay $375 million to resolve Justice Department allegations it promoted Botox for headache, pain and juvenile cerebral palsy from 2000 to 2005 without FDA approval."
        CNN (10/15, Watkins) reported, "The drug -- whose generic name is onabotulinumtoxinA -- has not been shown to work against migraines that occur 14 days or fewer per month, nor has it been shown to work for other forms of headache." The Wall Street Journal(10/15, Rockoff, Solsman) also reported the story.

A prospective, randomised, double-blinded trial to study the efficacy of topical tocotrienol in the prevention of hypertrophic scars

Khoo TL et al. – Despite widespread beliefs regarding the use of topical tocotrienol in the prevention of hypertrophic scars, there is very little evidence from well controlled and randomised clinical trials to justify its benefits for surgical scars. Twice–daily application of 5% topical tocotrienol had no significant effect on the appearance and vascularity of scars over 4 months post–surgery. Laser doppler imaging has a promising role as a scar assessment tool.

Enhanced Eyelashes: Prescription and Over-the-Counter Options
Aesthetic Plastic Surgery, 09/02/2010 Jones D – Originally indicated for the reduction of intraocular pressure, the synthetic prostaglandin analog bimatoprost was recently approved for the treatment of hypotrichosis of the eyelashes. In a double–blinded, randomized, vehicle–controlled trial, bimatoprost safely and effectively grew natural eyelashes, making them longer, thicker, and darker. Bimatoprost was generally safe and well tolerated and appears to provide an additional option for individuals looking to improve the appearance of their eyelashes.

We have noted improvement in my office staff and in my patients of their eyelashes with the use of Latisse.

Law requires New York hospitals to inform women undergoing mastectomies about reconstructive surgery.

The AP (8/18) reports that "a new law in New York will soon require hospitals to inform women who undergo mastectomies about reconstructive surgery and where to get it." The law "requires women to be informed of the various reconstructive options, and the advantages and disadvantages of each."
It is about time someplace sees this as critical  to the mental and physical health of women.

Laughter is the Best Medicine
(Ivanhoe Newswire) -- Repetitive mirthful laughter – or Laughercise  -- enhances your mood, decreases stress hormones, enhances immune activity, lowers bad cholesterol and systolic blood pressure, and raises good cholesterol (HDL).
 
Norman Cousins first suggested in the 1970s the health benefits of humor and associated laughter. His ground-breaking work documented his use of laughter in treating himself into remission from an autoimmune disease. His personal research was published in the New England Journal of Medicine,and he is considered one of the architects of mind-body medicine.
 
Dr. Lee S. Berk, director of the molecular research lab at Loma Linda University's Schools of Allied Health (SAHP) and Medicine, Loma Linda, CA, and Dr. Stanley Tan have picked up where Cousins left off.
 
Since the 1980s, they have been studying the human body's response to mirthful laughter. They established that laughter helps optimize the endocrine system, reducing stress by decreasing levels of cortisol and epinephrine. They demonstrated laughter’s positive effect on modulating components of the immune system, increasing production of antibodies and activating the body's protective cells.
 
Their studies have shown that repetitious "mirthful laughter," which they call Laughercise, causes a body response similar to the body response to moderate physical exercise.
"We are finally starting to realize that our everyday behaviors and emotions are modulating our bodies in many ways," Berk was quoted as saying.
 
Berk, Dr. Jerry Petrofsky and colleagues have completed a new study, which expands the role of laughter even further. They recruited 14 healthy volunteers to examine the effects that eustress (mirthful laughter) and distress have on modulating the key hormones that control appetite.
During the study, each subject was required to watch one 20-minute video that was either upsetting (distress) or humorous (eustress) in nature. The volunteers waited one week after watching the first video to eliminate its effect, then watched the opposite genre of video.
Researchers used the tense first 20 minutes of the movie “Saving Private Ryan” for a distressing video clip. This highly emotional video clip is known to distress viewers substantially and equally.
The volunteers were allowed to "self-select" the eustress video clips that most appealed to them to guarantee their maximum humor response. They were offered a variety of humorous options including stand-up comedians and movie comedies.
 
The researchers measured each subject's blood pressure and took blood samples immediately before and after they watched the respective videos. Each blood sample was examined for the levels of two hormones involved in appetite, leptin and ghrelin.
Researchers found that the volunteers who watched the distressing video showed no statistically significant change in their appetite hormone levels during the 20 minutes they spent watching the video.
 
In contrast, the subjects who watched the humorous video had changes in blood pressure and also changes in the leptin and ghrelin levels. The level of leptin decreased as the level of ghrelin increased, much like the acute effect of moderate physical exercise that is often associated with increased appetite.
 
"The ultimate reality of this research is that laughter causes a wide variety of modulation and that the body's response to repetitive laughter is similar to the effect of repetitive exercise,” Berk explained. “The value of the research is that it may provide for those who are health care providers with new insights and understandings, and thus further potential options for patients who cannot use physical activity to normalize or enhance their appetite."
For example, many elderly patients suffer from what is known as "wasting disease." They become depressed, lose their appetite and jeopardize their health. Based on Berk's current research, these patients may be able to use Laughercise as an alternative, initially less strenuous, activity to regain their appetite.
 
A similar loss of appetite is seen in widowers who suffer depression after the loss of a spouse. This can result in decreased immune-system function and subsequent illness. Chronic pain patients suffer from appetite loss due to the chemical changes in their body caused by intolerable discomfort.
 
While laughter may seem unimaginable in the face of deep depression or intense chronic pain, it may be an accessible starting point for these patients to improve and enhance their recovery to health.

Botox reduces wrinkles even in less frequent doses: OHSU research shows patients can reduce frequency of Botox Cosmetic treatments over time, saving money while still reducing dynamic wrinkles

PORTLAND, Ore. – Patients can decrease the frequency of Botox© Cosmetic injections after approximately two years and still receive most of the same wrinkle-smoothing cosmetic benefits, according to new research at Oregon Health & Science University.

"After two years of treatment at recommended intervals, patients can potentially cut the frequency, and thus the cost, of their Botox© treatments by half," said Roger A. Dailey, M.D., F.A.C.S., professor and Lester Jones Endowed Chair of oculofacial plastic surgery in the OHSU School of Medicine. The results of Dailey's work were presented at a meeting of American Society of Aesthetic Plastic Surgeon on April 24 in Washington, D.C. The research was sponsored by an unrestricted educational grant from Allergen, Inc., the maker of Botox© Cosmetic.

The Botox© research effort also demonstrated that the injections have a wrinkle preventing – or prophylactic – effect. Patients who begin receiving injections between their 30s and 50s are able to prevent wrinkles from forming and eliminate existing wrinkles, said Dailey, head of the Casey Aesthetic Facial Surgery Center, which opened in 1991 as part of Casey Eye Institute.

Based on previous studies, doctors advised patients who wished to reduce wrinkles in the glabellar region – the area between the eyebrows – that they needed to have Botox© Cosmetic injections every three months to maintain the cosmetic wrinkle-smoothing benefits. Such frequent treatment, however, deterred some patients, Dailey said.

Dailey studied 50 women ages 30 to 50, who received regular Botox© injections for two years. "We found that after the patient receives Botox© Cosmetic injections every four months for two years, the frequency of the injections can be changed to every six months and still achieve good results," Dailey said. "This demonstrates patients have the ability to achieve good results with broader treatment schedules and ultimately at a lower overall treatment cost.
Botox© has been approved for cosmetic use for eight years. In 2008, more than 5 million patients in the United States received cosmetic
Botox© treatments, according to Allergen, the manufacturer. About 313,000 of those patients were men.

Note: I have been using Botox for over 15 years for my staff and patients. Different strokes for different folks. Some patients require every 3 month treatment, some may have residual effects for as long as 6 months, I suspect some patients may eventually experience muscle atrophy from disuse and may need less as time goes on. Non-melanoma skin cancer cases increasing.
The CBS Evening News (3/15, story 6, 0:20, Couric) reported that "there has been a dramatic rise of certain types of skin cancer in older Americans. A new study of people on Medicare found that in just four years the number of procedures to treat non-melanoma skin cancers jumped about 77%. Researchers say the rise of sunbathing and tanning after World War II may have contributed to the increase."

Bloomberg News (3/16, Ostrow) reports, "The number of Americans treated for non-melanoma skin cancer increased 14.3 percent from 2002 to 2006, according to" a study published March 16 in the Archives of Dermatology. In 2006 alone, researchers "estimated there were more than
3.5 million non-melanoma skin cancers in the US...and about 2.1 million people were treated for the malignancy that year." In a March 12 interview, dermatologist and study author Howard Rogers, MD, PhD, stated, "There's an epidemic of skin cancer," which he attributed to a continued "lack of appreciation of the danger of going out in the sun."

The Los Angeles Times (3/15, Kaplan) "Booster Shots" blog reported that even though non-melanoma skin cancer "usually isn't deadly if found early," the economic toll of both basal cell and squamous cell skin cancer is "still significant. The American Academy of Dermatology says that treatment of non-melanoma skin cancer cost $1.5 billion in 2004."

HealthDay (3/15, Goodwin) reported that in another study published March 16 in the Archives of Dermatology, researchers "developed a mathematical model to estimate the prevalence of non-melanoma skin cancer in the United States." They discovered that "more people have had non-melanoma skin cancer than all other cancers combined over the last 31 years." And, "in a third paper in the same journal, researchers  from the US National Cancer Institute found survivors of one melanoma are about nine times as likely as the general population to develop a second melanoma."
Note: Skin cancers are increasing of the face because of the exposure to the sun. FDA releases updated safety data, allows silicone breast implants to remain on market.

NBC Nightly News (6/22, story 7, 1:25, Williams) reported, "Five years after they were allowed back on the market in this country, the FDA is saying they are confident [silicone breast implants] are indeed safe, although the risks are real."
       
The Wall Street Journal (6/23, Dooren, Burton, Subscription Publication) reports that on Wednesday, the Food and Drug Administration announced that early findings from an analysis of safety issues and adverse events associated with silicone-gel breast implants revealed no new concerns. Therefore, the agency said it will allow the devices to remain on the market. The Journal quotes the FDA as saying in the announcement, "Despite frequent local complications and adverse outcomes, the benefits and risks of breast implants are sufficiently well understood for women to make well informed decisions about their use."
       
The Los Angeles Times (6/23, Mai-duc) notes that the FDA was "criticized by consumer groups in 2006 for approving the implants after a 14-year hiatus because of widespread fear that they caused a range of illnesses." But, the FDA said the analysis released Wednesday, "indicates that they have a 'reasonable assurance of safety and effectiveness' when used properly. ... 'What we can say is the current data doesn't indicate that silicone gel-filled breast implants are linked to breast cancer, connective tissue disease or infertility,'" FDA's Center for Devices and Radiological Health Director Dr. Jeffrey Shuren "said in an interview." The Times points out that the analysis was conducted by implant manufacturers: "Santa Barbara-based Mentor Worldwide and Irvine-based Allergan Inc., which were required to conduct post-approval studies on more than 80,000 women over 10 years."
       
The National Journal (6/23, Brownstein, Subscription Publication) reports that Wednesday's analysis summarizes "four years of data after the 2006 approval of the implants, which had been scrutinized for evidence they could leak and cause cancer, fibromyalgia, rheumatoid arthritis, and a range of other ills." The study is expected to continue until 2016.
       
The AP (6/23, Perrone, Neergaard) reports that the FDA also made several cautionary statements. For one, "don't expect breast implants to last for life": The agency warned Wednesday that about "20 percent to 40 percent of patients who have implants for cosmetic reasons will need another operation to modify or remove them within eight to 10 years." For reconstruction patients, the "number is even higher at 40 to 70 percent." The FDA also emphasized that women making decisions on the gel implants need to understand that they come with complications, including "painful scar tissue and ruptured implants." Other problems include implant "wrinkling and a lopsided appearance."
       
Additionally, Bloomberg News (6/23, Larkin) reports that the FDA on Wednesday "reiterated its warning from January that as many as 60 of the five million to 10 million with breast implants globally have developed a rare type of cancer called anaplastic large cell lymphoma. This risk is 'profoundly small,'" Dr. Shuren stressed.
